What is MyChart?

MyChart isa secure, online tool that allows patients to connect with their personal health information 24/7.Patients can access MyChart from any Internet connected computer or smartphone device.

With MyChart you can...

  • Request an appointment with your Warren Clinic or Laureate physician's office

  • eCheck-In before a scheduled outpatient appointment

  • View your lab and test results

  • View medication information and request a prescription refill from your Warren Clinic or Laureate physician's office

  • Review and pay your bill online

  • Communicate with your Warren Clinic physician's office

  • Manage health information for your family members (permission required)

  • View health education topics

  • Receive and accept earlier appointment offers when you’re on a waitlist

How do I sign up?
First, you will need an activation code. This code will enable you to login and create a personal user ID and password.

  1. If you have received an activation code from your doctor's office, click on the "I have an activation code" link on the right side of this page.

  2. If you do not have an activation code, please click on the "Request an activation code" link in the same area and one will be emailed to you.

  3. Enter your activation code and follow the prompts to complete your MyChart enrollment.

What technology do I need to log into MyChart?
You need access to a computer connected to the Internet and an up-to-date browser.

Is there a fee to use MyChart?
MyChart is a free service offered to Saint Francis Health System patients.

Is MyChart secure?
Access to information is controlled through secure activation codes,personal IDs and passwords. Each person controls their password, and the account cannot be accessed without that password. MyChart uses the latest encryption technology to secure the integrity of your account. Unlike conventional email, all MyChart messaging is done while you are securely logged on to the website.

Which physicians may I make appointments with in MyChart?
You may use MyChart to schedule appointments only with physicians who have previously treated you while using our new electronic medical record. Appointments with a new physician may be made through referral, by calling your physician’s office or by calling the Warren Clinic Referral Line at 918-488-6688.

What is eCheck-In?
eCheck-In allows you to check in online before a scheduled outpatient visit and reduces the need for completing additional paperwork during your visit. Several days prior to your appointment, you will receive an email or text reminder to complete eCheck-In.

How do I receive appointment offers when I'm on the wait list?
When you’re on the wait list, you’ll automatically receive notifications when an earlier appointment slot opens up. Automatic waitlist offers can be sent up to 5 patients at once, so it’s important to sign into your MyChart account to review any offers at your earliest convenience. If you miss an offer, don’t worry – you’ll keep your place on the wait list for additional offers in the future. You can easily add yourself to the wait list by looking at any future appointment on your Visits page.

When can I see my test results in MyChart?
Saint Francis MyChart allows you to see lab test results and imaging reports at the same time they are released to your provider. This includes access to laboratory reports, pathology reports, genetic testing information and more.

If I send a message to my physician or nurse, when can I expect a reply?
You will generally receive an answer within 1–3 business days. Please note that MyChart should not be used for urgent situations. Please dial 911 if it is an emergency.

Will my entire medical history be made available in MyChart?
MyChart is a direct link between you and the information in your electronic medical record.It will not reflect your complete medical history, though some patients may see past records in order to ensure continuity of care and access to information about ongoing treatments.

Any appointment can be viewed under Visits> Past Appointments.

Can I ask my primary care physician for medical advice through MyChart?
Yes. You can send a secure message to your Warren Clinic physician if you have seen them since enrolling in MyChart.

Can I view a family member’s health record in MyChart?
Yes, this is called proxy access and allows a parent (or legal guardian) to log into their personal MyChart account, and then connect to information regarding their child and/or teen. Adult patient information may be accessed by family members with permission of the patient, or if the patient is incapacitated, guardians, powers of attorney or other legally authorized representatives may be allowed to access information of the patient on MyChart. Can I ask questions regarding a family member from my MyChart account?
MyChart offers direct access to your personal health record. If you aska question about a family member, the response would appear in your health record and not in the family member’s health record, potentially jeopardizing your family member’s medical care. We recommend asking questions only about the patient who is currently logged in to MyChart or as a proxy on behalf of the family memberwhile accessing their MyChart account.

Can my spouse and I share one MyChart account?
No, each adult must establish their own MyChart account.

If some of my health information on MyChart is not correct, what should I do?
Your MyChart information comes directly from your electronic medical record at your physician’s office or the hospital. Contact your physician’s office to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

Is my activation code my user ID?
No, your activation code is not your MyChart ID or password. You will use this code only once to log into MyChart for the first time. When you log into MyChart the first time, you will then be asked to create your own unique MyChart ID and password.

Can you send me a new activation code as I have lost itor did not receive it?
Yes. Please contact the MyChart team for a new code.

My activation code does not work; what should I do?
Please contact the MyChart team for assistance.

Where can I update my personal information (e.g., home address, email or change my password)?
Log in to MyChart and go to the "Preferences" section and select the appropriate option.

I forgot my password. What should I do?
Visit the Saint Francis MyChart site and click the "Forgot Password" link on the sign-in page to reset your password online.

What is MyChart’s Privacy Policy?
MyChart is fully compliant with federal and state laws pertaining to your privacy. Your name and email address will be treated with the same care and privacy given to your health records.
